Prior To Your Initial Appointment:

Please call us or email us with your name and email address so we can send you an intake packet with all the necessary forms. 

These forms need to be filled completely-legibly and faxed to the office. Appointments will only be scheduled upon receiving all paperwork including insurance information and notes from your referring Providers.

Please note that this is the only way to ensure that you make the best use of your time with the Doctor. 

Please bring the originals with you to your initial appointment as some faxes may not be legible. 

If you have been seen by another Psychiatrist or Psychologist or been in the hospital, please download, sign and fax to us the from labeled "Medical records release" at least a week prior to your appointment so we can obtain all your relevant records. 

NB: emailing of forms containing personal health information are prohibited by HIPPA regulations.
